| In
March of 1995, The Arc, Oneida-Lewis Chapter opened the Article 16 Clinic,
ClinicWorks, located at 14 Arnold Avenue in Utica, New York.
ClinicWorks is a joint venture with Central New York DSO in Rome,
New York. The Lewis County clinic of ClinicWorks
opened in October of 1997 in Lowville, New York. ClinicWorks is
a New York State Office of Developmental Disabilities (OMRDD) certified
Article 16 Clinic which provides therapeutic rehabilitative and habilitative
services to individuals (children and adults) with developmental disabilities
in Oneida and Lewis Counties.

All
our clinicians have had specialized training in the mental retardation/
developmental disabilities field and are certified and licensed to practice
in the State of New York. All services are monitored under the supervision
and approval of a qualified physician (Medical Director). All individuals
referred for services are assigned a counselor or coordinator who serves
as the primary contact person and is responsible for case coordination.

Who
is eligible
 | Adults with
a documented developmental disability. |
 | Adults and
children who are in need of an assessment and evaluation for the purpose
of determining a developmental disability. |
 | Children under
the age of five whom are at risk or where there is a substantiated suspicion
of a developmental disability. |
 | Children six
years and older who are in need of clinical assessment to substantiate
a specific developmental disability. |
Who
is not Eligible for Services
Individuals
who are enrolled and attending a Day Treatment program or who live in
an ICF program are not eligible for services through the Article 16 Clinic
except for Vocational Rehabilitation Counseling services.
Necessary
Documentation
The following
documentation is required for admission:
 | Psychological
and or medical document that establishes a developmental disability. |
 | A current physical
(within the last year) |
 | A current mantoux
(PPD) test (within the last year) |
 | Completion
of the clinic intake process |
 | Written approval
from the ClinicWorks Medical Director |
Admissions
Process
The
admissions process has four basic steps:
 | Referral and
intake: A meeting will be scheduled to discuss clinic services and need
and to have all required papers and documents signed. |
 | Assessment:
Assessment(s) will be scheduled for requested services following the
intake |
 | Treatment meeting:
Once all assessments are completed, a treatment meeting will be held
to discuss the treatment plan of services. |
 | Treatment phase:
Once the treatment plan is signed, therapy begins. |
How
to Make a Referral
 | A simple phone
call to the clinic office will begin the process. Have ready the following
information: |
- Referral’s
name
- Address
- Phone number
- D.O.B.
- Residential
information: contact person
- Insurance information
- What services
you are requesting
 | You will be
given an intake date and the name of the counselor assigned to this
individual. |
 | In most cases,
you will be given the assessment date(s) as well. |
 | A letter will
follow to remind you and the individual of the meeting and what documents
to bring with you.

Evaluation and Treatment
 |
All individuals
are evaluated in ate area(s) they are referred for. |
 |
When evaluation(s)
are completed and treatment is necessary, a Treatment Plan is
developed and reviewed at a formal meeting. |
 |
Once the Clinic
Medical Director approves the Treatment Plan, services can commence. |
 |
Therapy(s) is
scheduled according to the treatment plan. |
Discharge
Criteria
 | Services are
no longer necessary. |
 | Admission
requirements to remain eligible are no longer met. |
 | The individual
and/or the collateral no longer wish to receive services. |
 | Failure to
show for appointments over a six month period. |
In the event an
individual is not approved for clinic services, the individual and the
referral source will be notified of the reason why services were
denied.

and Medicare are accepted.
