A Medicaid Service Coordinator (MSC) is a qualified professional who provides assistance, support and guidance to people with developmental disabilities. An MSC helps people strive for the highest quality of life, focusing on assisting each person to achieve his or her unique goals and desires including but not limited to: choice of home; meaningful work and/or community activities; social and leisure activities; meaningful relationships; and access to services and supports that promote optimal health.

Service Coordinators promote the concept of informed choice, individualized services and supports, individual satisfaction through the use of a person centered planning approach in assisting in developing , implementing and maintaining an Individualized Service Plan.

As one of the largest providers of Service Coordination in the Central New York area, we are committed to providing individuals and families with tailored service delivery based on an individual’s needs and life goals. We want to ensure an individual’s health, safety and a great quality of life.

Examples of what Service Coordinators do:

  • Help the person with developmental disabilities make informed choices
  • Focus on the aspirations of the person and assist with reaching his or her personal goals
  • Promote activities which will increase the individual’s independence and life satisfaction
  • Advocate for individuals in all settings to protect and uphold their individual rights
  • Take reasonable steps to ensure the individual’s health and safety needs are met
  • Assist in securing and maintaining benefits and resources (i.e., Medicaid, SNAP, Social Security, etc.)
  • Find, refer and assist with securing needed waiver services or non-waiver services that will meet individual needs
  • Monitor and evaluate services with the person and their support team
  • Make referrals for various housing options and residential services
  • Assist individuals in having meaningful community recreational and social activities
