The Arc is able to provide coordination of services for adults and children with disabilities.

Welcome to Service Coordination

The Arc Service Coordination Division offers four program options in Oneida, Lewis, Herkimer, Madison, and Jefferson Counties.  Each option offers unique opportunities to a diverse population of individuals. The Arc provides SC to individuals with intellectual and developmental disabilities, traumatic brain injury, and the elderly.

Service Coordination programs:

  • OPWDD Medicaid Service Coordination
  • OPWDD Plan of Care Support Services
  • NYSDOH Traumatic Brain Injury - Waiver Service Coordination
  • NYSDOH Nursing Home Transition and Diversion -  Waiver Service Coordination

A Service Coordinator is a trained professional who assists individuals in gaining access to an array of services and supports that will enable them to lead a more fulfilled life according to his/her particular needs and desires.

Service Coordinators promote the concept of choice, individualized services and supports, individual satisfaction through the use of person centered planning process in assisting in developing , implementing and maintaining an Individualized Service Plan.

A Service Coordinator will schedule a monthly meeting to provide a variety of services. 

Examples of things Service Coordinators do:

  • Is responsible to the individual and assist with identifying his/her unique needs
  • Promote activities which will increase the individual’s independence and life satisfaction
  • Advocate for individuals in all settings
  • Take reasonable steps to ensure the individuals health and safety
  • Assist in securing and maintaining benefits and resources (ex: Medicaid, SSI, etc)
  • Find, refer and assist with securing needed waiver services or non-waiver services that will meet individual needs
  • Monitor and evaluate services with individual and their support team
  • Make referrals for various housing options and residential services
  • Assist individuals in having meaningful community recreational and social activities
