Awake-Over-Night Manager Float

You will be responsible for ensuring the health and safety of the individuals. Also, you will be responsible for the effective and efficient physical and social operation of the residence.

Schedule is overnights 10pm-8am various days of the week, Various houses in Oneida County

Responsibilities:

  • Assure the maintenance, housekeeping and food preparation duties of the residence in order to provide a clean, neat, healthy and safe environment.

  • Know the current Individualized Service Plan specific to each individual and follow through with the provision of services as outlined.

  • Develop viable solutions to problems and needs concerning individuals, staff, and the day to day operation of the residence. Participate in all training as required.

Qualifications:

  • High School Diploma

College Works Resident Assistant

You will be accountable for the health and safety of the individuals in the supported apartments (Dorm Rooms) and for maintaining the physical and social operation of the supported apartments.

Per Diem; Variable Schedule. Mostly weekends/evenings

Responsibilities:

  • Assist the individuals by providing oversight, guidance and training in the completion of daily living tasks.

  • Assist in household tasks such as laundry and housecleaning.

  • Assist in menu planning, preparation and cooking, ensuring that the individual’s dietary and nutritional needs are followed. Shopping with the goal of increasing independence.

  • Medical and dental appointments with oversight by the IRA RN. Includes completing health care records, act as liaison with medical professional and administer medications as required.

  • Assist with Financial management, budgeting, banking and shopping.

Qualifications:

  • High School Diploma or G.E.D.

  • One year documented experience working with people with disabilities. (work as a college RA beneficial)

Day Habilitation Specialist I

You will provide assistance and supervision of the individual’s day-to-day activities, socialization, recreation and daily living skills development to improve the individual's quality of life.

Schedule is Monday-Friday 8:30 - 4PM; 35 Hours

Responsibilities:

  • Day to day implementation of assigned person we support’s Individualized Service Plan ( ISP/Day Habilitation Plan) utilizing as many appropriate resources as possible in the community
  • Ensuring supervision of assigned persons at all times
  • Promote and follow person driven choices in independence, individualization, inclusion, and productivity
  • Develop and maintain a positive relationship with the person we support, working with the person and other team members to help determine and achieve valued outcomes for the individuals

Residence Counselor Awake-Over-Night

You will provide management of the night operations of the residences and supervision of the individual during night period. Light housekeeping duties apply.

Residence Manager

You will provide management of the day-to-day operations of the residences; supervision of the individuals day-to-day activities, socialization, recreation and daily living skills development.

Full Time & Part Time; Variable 3 day incremented schedules, sleeping overnights required.

Responsibilities:

  • Assure the maintenance, housekeeping and food preparation duties of the residence in order to provide a clean, neat, healthy and safe environment. Provide oversight and guidance if necessary to the individuals in the completion of household tasks such as laundry, menu preparation, and shopping, cooking, minor maintenance of the house and grounds with the goal of increasing independence.
  • Know the current Individualized Service Plan specific to each individual and follow through with the provision of services as outlined.
  • Review the Daily Notes and House Log since their last shift. Complete or review incident reports and forward to the Program Manager within prescribed time frames. Assure that medication verification is completed per policy during their shift. Follow fiscal accountability procedures and maintain accurate financial records for each individual.

Respite Specialist

You will be providing respite (relief) for family members of individuals with intellectual and other developmental disabilities.

Full Time; Awake Over Night 35 Hours, Care Home

Responsibilities:

  • Implementation of the plans and activities provided by the supervisor of the assigned program.
  • Ensuring supervision of assigned persons at all times
  • Becoming familiar with the individuals’ safeguards and provide any change of information and/or concerns to the program’s coordinator.

Sr. Respite Specialist [Associates Degree]

You will be responsible for the immediate supervision of the program, including protective oversight of the individuals, implementation of the plans and activities, staff direction and supervision, and physical plant safety. The Senior Respite Specialist will also perform direct care as needed.

Full Time; Awake Over Night 35 Hours, Care Home

Responsibilities:

  • Become familiar with the individuals Care Plan/Safeguards and provide any change of information and/or concerns to the program supervisor.
  • Plan and participate in integrated activities that are accessible and rewarding to the individuals in the program.
  • Accurate and timely submission of documentation as assigned. (Ex. Daily notes, attendance, care assignments, inventories, ledgers, logs and timesheets. Ensure daily documentation requirements are met and that information is correct and complete prior to submission.
  • Administer medications (site based only) per NY State and The Arc Oneida – Lewis Chapter policies and procedures.
  • Participate in activities of daily living as necessary, i.e.: toileting, eating, dressing, bathing, and ambulating to whatever extent is required based on individual needs, including lifting assistance as needed.

Accounting Manager of Procurement [Bachelor's Degree]

You will meet the goals and objectives for the procurement of goods and services for the agency. Perform and maintain the related accounting processes in accordance with generally accepted accounting principles (GAAP). This position supervises the Purchasing/Inventory and Accounts Payable functions in the Financial Management department.

Daytime Program Manager [Bachelor's Degree]

You will be responsible for the health and safety of the people we support, responsible for the effective and efficient physical and social operation of the residence and responsible for the development of positive relationships with neighbors, local businesses and other service providers. Oversee the staff in the implementation of the Individualized Service Plan.

Schedule is primarily Monday-Friday but flexible to include nights, weekends and on-call needs.

Responsibilities:

  • Develops the residential habilitation plans including all appropriate assessments, supervises and trains the staff in the implementation of habilitation service plans in accordance with the agency’s mission and state regulation.

  • Assures that evacuation and fire safety issues are addressed within the OPWDD guidelines including assessment of evacuation capability of individuals

  • Supervise and monitor all aspects of running and operating the residence including timely submission of work orders, maintenance, and upkeep of the home, and recommending replacement of equipment and furniture as needed.

Qualifications:

Plan Writer [Bachelor's Degree]

You will be responsible for overseeing the plan of services based on person centered planning concepts that are delivered to each assigned person we support. The Plan Writer works with the person and their support team to develop and revise the habilitation goals with a focus on areas that are most important to the person, and is ultimately responsible for ensuring consistent implementation and documentation of all aspects of the individual’s program plan. The Plan Writer develops these services in compliance with all applicable state and federal regulations.

Full Time; Monday – Friday, 35 Hours

Responsibilities:

  • Develop all aspects of the Staff Action Plan, including IPOPs & Safeguards at least annually for each person we support assigned using person centered planning. Staff Action Plans must also be reviewed at least semi-annually.
  • Collaborate with departments to ensure all information necessary for new intakes is obtained, implemented and, shared appropriately with team members.
  • Attend annual and semi-annual review meetings for each individual assigned. Ensures appropriate follow up on recommendations.
  • Work closely with the person and their support team to ensure services, health related and otherwise, are implemented appropriately.
  • Develop monthly summaries for services each person receives. Analyze data and determine progress, regression, etc. Revise program plan as needed.
  • Participate in decisions-making process related to people assigned as appropriate.
  • Conduct on-site observations and provide feedback to staff to ensure proper, consistent implementation of goals and plans.

  • Assists the Registered Nurse in overseeing the general health and routine medical management of all program residents on a daily basis to include basic therapeutic and preventative nursing procedures
  • Assists direct care staff with extraordinary comprehensive medical appointments, emergency room visits and hospital stays.
  • Assists with nursing assessments, summaries and medical records under the supervision of the Registered Nurse.
  • Provide guidance and instruction to staff and individuals on diet and nutrition.  Reviews menus for proper nutrition.

Responsibilities:

  • Assist Program RN in the management of illnesses and accidents.
  • Assist Program RN in the provision of nursing services.
  • Assist Program RN in medical record keeping, nursing assessments, monthly summaries and nurse’s notes.
  • Administer medication as assigned.
  • Assist in monitoring program participants’ nutritional status.

Qualifications:

Part Time 21 Hours; Schedule is flexible or standard Monday-Friday

Responsibilities:

  • Monitor medication safety, storage and administrations via AMAP personnel
  • Provide management of accidents and illnesses; making necessary referrals or recommendations to appropriate staff or others
  • Complete and maintain all required documentation, both in electronic and paper format
  • Provide professional and clinical nursing oversight of the LPNs and AMAP staff assigned to sites under your supervision. Provide directives on medical oversight to any Direct Support Professional

Qualifications:

Provide individualized services to students with disabilities to improve their outcomes after high school and assist with a positive transition from high school to adult life.  Such services may include but are not limited to : developing plans for work and life skills training and experiences based on the students Individualized Education Plan (IEP) and personal goals.

Full Time; Monday – Friday; 35 Hours

Responsibilities:

  • Complete referrals for students to obtain eligibility for ACCES-VR and/or OPWDD and attend meetings as necessary regarding the referrals.
  • Attend annual CSE meetings and school/student meetings and/or provide beneficial information to assist with such meetings. Also, other parent-teacher meetings, transition fairs and/or, but not limited to open houses, in order to promote and expand referrals for all STW programs for the current & upcoming school years.
  • Complete necessary documentation as it pertains to each student and distribute appropriately to their school personnel and family.
  • Accurately complete and maintain all required agency documentation in the required manner.
  • Provide classroom based instruction and community based learning experiences based on the students Individualized Education Plan (IEP) and personal goals.
